Sheriff’s office launches Developmental Disabilities Form

With Autism Acceptance Month beginning on April 1st , the Kosciusko County Sheriff’s Office has announced the release of a new Developmental Disabilities Registration Form for First Responders.

This project is not exclusive to Autism Spectrum Disorders and can be utilized for any applicable disability. The form is available for download here. The return instructions are located in the final paragraph of the documentation.

Once the PDF fillable form has been submitted to our communications center, it will be attached to readily accessible name and address records. Users may also send a photograph of the individual, if they choose. The purpose of this new feature is to aid responding personnel with potentially critical details in the event of an emergency.
