A Warsaw man is facing charges of deception of an identity.

28-year-old Joseph Lambert was arrested Monday.

According to a probable cause affidavit, a local woman received a letter dated April 18 stating an application for an American Express Gold Card using her identity had been denied.

According to the affidavit, the victim told police she believed Lambert was the one applying for the card using her information and he had written down her information when selling her insurance.

On May 2, the victim went to WPD a second time with two more letters stating credit card applications using her information had been denied – one from Wells Fargo and one from Capital One.

When she contacted Wells Fargo, the victim claims she was informed all her information was used in the application for the credit card except her address, email and phone number.

The affidavit states Chuck Hodges of the Warsaw Police Department received a letter from the victim with the credit card application from Wells Fargo using her information and listing Lambert as an authorized user.

Lambert is currently booked at Kosciusko County Jail with bond set at $5,250.
